TRANSFORMATION investigators helped develop new Health Quality Ontario tool, the primary care Patient Experience Survey. This standardized survey is an evidence-based tool that will help providers develop a deeper understanding of their patients’ experiences of primary care. The survey is aimed at improving patient experiences and helps providers see their practices through their patients’ eyes.
